About the Programme

The programme is for people who have had a recent heart attack, angioplasty or heart surgery or those who have had a recent diagnosis of CHD or Angina. We offer a choice of a group programme at Surbiton Hospital or a home-based programme using the Heart Manual.

Patients must either live within the borough of Kingston-upon-Thames or have a GP within the borough

What to expect
when you come

We offer: A menu of rehab options aiming to support people to self-manage their condition, recover from an acute cardiac event and optimise health. This is through an in-depth initial assessment followed by an individualised plan of rehabilitation via a range of face to face appointments, telephone or video consultations, face to face exercise classes, online exercise classes, online education programme, medicines review clinic. On completion of the programme people are discharged to longterm cardiac care with their GP team who will receive a detailed summary of their rehab care. The team is made up of specialist nurses, physio, specialist exercise instructors and a dietician.

How long does it take

Each session is 2 hours long, but pts may attend all or some of each session.
