About the Programme

Cardiac Rehabilitation Gwynedd and Anglesey North Wales.
The team was established in 2001 and comprises of Cardiac Rehabilitation
Specialist Nurses, an Exercise Physiologist, a Part Time Occupational Therapist and
a full time secretary.
The area covered by the team is mixed urban and rural. We support the following
conditions:
STEMI
NSTEMI
ACS
PCI - stents
Valve Replacement
CABG
Heart Failure / cardiomyopathies via the heart failure team
Arrhythmias via the arrhythmia specialist nurse
Transplant
Devices (ICD, pacemaker, Bi-vent)

What to expect
when you come

In hospital the majority of patients are seen by the ACS nurses who provided information regarding event, pharmacological treatment and planned intervention. If the patients stay in hospital is prolonged then a team member will visit them there.

Almost all patients will have a home visit to ensure a thorough assessment of patient and environment which ensures that advice and support given is appropriate to each individual.

Classes run at Ysbyty (HOSPITAL) Gwynedd, Tywyn, Barmouth and Holyhead leisure Centre Anglesey all classes run for an 8 week rolling program. Again due to the rurality of the area the team make full use of the community leisure services gyms and have a very close working relationship with our trained leisure services instructors who provide and run the community classes.

Every leisure centre within Gwynedd and Anglesey has a phase 4 class running twice a week for 16 weeks. Following completion of these classes patients are referred on to the national exercise referral scheme (NERS).
