About the Programme

The Multidisciplinary team is based at the University Hospital of Wales, Cardiff. The Cardiac Unit at the University Hospital of Wales is a tertiary referral centre receiving patients from South East Wales. The Cardiac Rehabilitation team visit the Cardiac wards on a daily basis to collect patient referrals. Patients who fit the referral criteria are seen by the Cardiac Rehabilitation nurse and then a referral is made to the local Cardiac Rehabilitation team if out of Cardiff area.

The Cardiac Rehabilitation programme which consists of education, exercise and stress management sessions will help to:-

• Understand the condition
• Recover from heart attack or surgery
• Make lifestyle changes to help improve heart health
• Reduce the risk of further heart problems

The Cardiac Rehabilitation programme is offered in 3 venues to improve access for all patients. The University Hospital of Wales offers two programmes, Llandough Hospital and Gabalfa Hub one programme each. The programmes are held twice a week for six weeks. The topics covered in the education programme include, understanding your condition, information on nutrition, medication, stress management, physical activity, cardiac risk factors –cholesterol/blood pressure and information on holidays /travel and returning to work.

What to expect
when you come

The Cardiac Rehabilitation programme is very informative in a relaxed and friendly atmosphere ensuring everyone feels welcome and enjoys the sessions.

There will be two trained Cardiac Nurses and a Cardiac Physiotherapist present at all sessions with educational input from Occupational Therapy, Dietetic and Pharmacy.

Prior to commencing the Cardiac Rehabilitation programme a pre assessment will be completed by the nurse and physiotherapist to review requirements and any limitations.

How long does it take

The programme starts approximately 4 to 6 weeks post events depending on recovery. The length of time of the each session depends on the venue but generally will last approximately 2hours.

General information

On completion of the Cardiac Rehabilitation programme patients can be referred to maintenance exercise classes across Cardiff and Vale run by the National Exercise Referral Scheme and also individually run exercise classes.
