About the Programme

**URGENT** Changes to referral emails from 18th October 2021**

Due to changes within the referral pathway for cardiac patients, please ensure that the correct referral email has been used. Patients should be over 18yrs old and registered with a GP in Ashton, Leigh and Wigan or be an unregistered permanent resident. The cardiac departments within WWL operate a non-urgent service within the office hours of Mon-Fri 9am – 5pm.

Confirmed diagnosis of myocardial infarction and ongoing treatments should now be sent to: wwl-tr.commcardiacnursing@nhs.net
If you need to speak to the Cardiac Rehabilitation Nursing Team or your referral is a priority call, please contact them on 01942 482230.

Revascularisation procedure without a precursor myocardial infarction (PCI, CABG, Valve, Angina etc) should continue to use this email wwl-tr.wwlcardiacrehab@nhs.net
If you need to speak to the team or your referral is a priority call, please contact them on 01942 264905.

What to expect
when you come

Cardiac Rehabilitation is a comprehensive programme to support patients physically and psychologically following a cardiac event. This includes risk factor / lifestyle advice. Cardiac rehabilitation can help patients/carers understand how best to manage heart health following an event.

Core cardiac rehabilitation integrates structured lifestyle interventions and medications, reduces morbidity and mortality among cardiac patients. It consists of a individual programme of exercise and education designed to help you return to optimal fitness and function following a cardiac event.

The patient is assessed, a care plan developed and a comprehensive cardiac rehabilitation programme delivered. A final cardiac rehabilitation assessment is conducted prior to discharge to long term management with GP.
