About the Programme
Phase I: In-patient visit from a Cardiac Rehabilitation Specialist Nurse. written and verbal information provided on heart attack, angioplasty and treatments including medication. Individualised planning to manage modifiable risk factors Referral to stop smoking specialst, alcohol specialist nurse , dietician and diabetic specialist nurse if required
Phase II: Home visits / nurse-led clinic offered within 5 working days of receiving referral Risk factor modification Medication review, management and adjustments as required Cholesterol and blood pressure control on an individual patient need. monotor and evaluate progress Phase III: Assessment and advise on activity and exercise incuding a 6 week rehabilitation exercise programme at Leigh Health Centre, Platt Bridge Health Centre or Wigan Health Centre as well as medicine management and Basic Life Support session. Stress management and relaxation sessions offered on a group or individual basis dependant on patient need and referral to Clinical Psychology where appropriate.
Phase IV: Referral to to Active living Team for exercise maintenance.
